Insurance

I am an in-network provider for:

• Anthem/Blue Cross Blue Shield

• IU Health

• CareSource

• United Behavioral Health

• Aetna

• Cigna

• Tricare

My billing service will submit insurance claims for you.  You will be responsible for paying any remaining deductible, then your co-pay or co-insurance fees.

If I am out-of-network with your insurance, I can offer a sliding scale fee. This ranges from $60 - $120 and is based on total gross household income.

Please be sure to contact your insurance company to understand your outpatient mental health benefits. This would include provider status, your deductible, and your co-pay or co-insurance.

I accept cash, checks, credit cards, health/flex savings accounts (HSA and FSA), Venmo and PayPal. You will receive a monthly statement from my billing service with your balance due.

Rates

• Initial Intake Session:  $140

• Individual Therapy Session:  $120

• Family Therapy Session: $120

Cancellation Policy

If you need to cancel an appointment, please let me know via phone, email, or text with at least 24-hours notice. If you do not show to an appointment or cancel with less than 24-hours notice, you will be charged a $60 late cancellation fee. Please note insurance does not cover these fees. Exceptions will be made for illness and emergencies.
